The Yaman ng Kalusugan Program (YAKAP) is PhilHealth’s expanded primary care benefit package that provides all members and their dependents access to free consultations, essential medicines, and diagnostic tests.
The purpose of YAKAP primary care is to act as the first point of contact for individuals within the healthcare system, focusing on proactive wellness rather than just reactive treatment. It shifts the healthcare focus toward prevention and early detection to keep people "malayo sa sakit" (far from illness). Any complicated, emergency, advance or terminally ill cases are NOT also covered by YAKAP.
Please note that the Functional & Lifestyle medicine service is NOT covered by Philhealth YAKAP.
Patient Obligations under YAKAP
As an empaneled beneficiary, you are responsible for the following:

1. Secure a PhilHealth Identification Number (PIN)
2. Register for the YAKAP Program
You must register and select a specific YAKAP-accredited clinic as your primary care provider using one of these methods:
3. Complete the First Patient Encounter (FPE)
4. Avail of Benefits during Consultations
5. Note on Fees: Services at public YAKAP clinics are completely free. Since our clinic is a private institution, you will be charged a maximum annual co-payment of ₱900 which will be paid upfront.
6. Transfer Eligibility & Timing
According to the latest PhilHealth YAKAP Clinic Transfer Guidelines, the timing for a transfer is as follows:

*The medicines above is provided by Brim Pharma/Mymed but will be dispensed at our clinic
*We decided to provide a two (2) weeks' supply each time for medications that address hypertension, diabetes, and cholesterol to avoid depleting the supply quickly. For refills, simply come back and bring your prescription 1-2 days before the last pills run out or 1 day after, provided that the schedule is available.
*Prescriptions not from our YAKAP doctor will not be accepted
*Medications not listed above will be referred to the GAMOT accredited pharmacy, if available, which will give you P20,000 yearly credit line.

*The amount of medicine will be deducted to the fixed fee schedule (i.e. P20,000 per member per year). The amount cannot be saved or carry-on for unused fee the following year, Also, it is is not a form of aid ("Ayuda") or cash.
*You can only get the medicines if your have the prescription coming from your YAKAP accredited doctor only

*Also, the targeted diagnostics are based on risk and age at health assessment not on the request of the patient as they pleased.






Note: Most of the collection of sample for lab test can be done at the clinic. Our medical technologist is available only Monday to Saturday 6 AM TO 6:30 AM ONLY. The 21 Esssential meds are to be dispensed at the front desk.
