About the Service
The Yaman ng Kalusugan Program (YAKAP) is PhilHealth’s expanded primary care benefit package that provides all members and their dependents access to free consultations, essential medicines, and diagnostic tests.
The purpose of YAKAP primary care is to act as the first point of contact for individuals within the healthcare system, focusing on proactive wellness rather than just reactive treatment. It shifts the healthcare focus toward prevention and early detection to keep people "malayo sa sakit" (far from illness). Any complicated, emergency, advance or terminally ill cases are NOT also covered by YAKAP.
Please note that the Functional & Lifestyle medicine service is NOT covered by Philhealth YAKAP.
Patient Obligations under YAKAP
As an empaneled beneficiary, you are responsible for the following:
- Data Accuracy & Updates: Ensure your member records in PhilHealth are updated and provide only true and accurate information to the clinic.
- Active Participation: Actively participate in the health care plan developed and agreed upon with your YAKAP physician.
- Continuous Engagement: Visit your chosen YAKAP clinic regularly for follow-up consultations, laboratory tests, and diagnostics to maintain continuity of care.
- Communication of Changes: Promptly inform the clinic of any relevant changes in your condition or circumstances that might affect how care is provided to you.
- Adherence to Protocols: Respect all clinic protocols and established treatment guidelines during your visits.
- Identity Verification: Undergo a mandatory PhilHealth Check Utility (PCU) Liveness Check during every patient encounter to validate your identity and confirm eligibility for benefits.
Step by Step Process to Avail the YAKAP Service
1. Secure a PhilHealth Identification Number (PIN)
- If you are already a registered PhilHealth member, proceed to registration.
- If not yet registered, apply for a PIN through the PhilHealth Member Portal or at any local PhilHealth office.
2. Register for the YAKAP Program
You must register and select a specific YAKAP-accredited clinic as your primary care provider using one of these methods:
- Mobile App: Download the eGovPH Super App, navigate to the PhilHealth section, and select Add YAKAP Provider. Select Nourished Living Medical Clinic
- Online Portal: Use the PhilHealth Member Portal to choose your clinic.
- In-Person: Visit any PhilHealth Local Health Insurance Office (LHIO) or go directly to your chosen YAKAP-accredited clinic for assisted registration.
3. Complete the First Patient Encounter (FPE)
- Visit your chosen clinic to undergo the First Patient Encounter (FPE). This is a baseline health assessment and identity verification process required before you can receive continuous benefits. The FPE is not the same as the medical consultation, wherein you have a complain or sickness.
- Bring a valid government ID and your Member Data Record (MDR).
- You will be verified through a PhilHealth Check Utility (PCU) Liveness Check which is a mandatory facial recognition process used to verify a member's identity and ensure they are physically present when availing of YAKAP services. In addition, you will sign a YAKAP Empanelment Slip (YES) to formalize your enrolment at that facility.
4. Avail of Benefits during Consultations
- Check-ups & Labs: Once registered, consultations, selected 13 laboratory tests and 21 Essential medicines are covered depending on the doctor's assessment & recommendation. The laboratory are outsourced to Singapore Diagnostic and BMCDC (Buhangin branch) whereas the 21 essential medicines are provided by Brim Pharma/Mymed. See below the details of the 13 laboratories and 21 Essential medicines.
- Medicines (GAMOT): If the doctor prescribes medication not in the 21 Essential medicines, they will issue a prescription with a Unique Prescription Security Code (UPSC). Present this code and your ID at any GAMOT-accredited pharmacy to claim free medicines (up to ₱20,000 yearly credit line). You can go to any accredited GAMOT facility in Davao nearest to you.
- Cancer Screenings: Free screenings like mammograms or colonoscopies are available at select hospitals with a valid doctor’s referral from your YAKAP provider.
5. Note on Fees: Services at public YAKAP clinics are completely free. Since our clinic is a private institution, you will be charged a maximum annual co-payment of ₱900 which will be paid upfront.
6. Transfer Eligibility & Timing
According to the latest PhilHealth YAKAP Clinic Transfer Guidelines, the timing for a transfer is as follows:
- Before the First Patient Encounter (FPE): If you have not yet undergone your FPE or availed of any benefits at your current clinic, you may request a transfer at any time. Once approved, the transfer takes effect immediately.
- After the First Patient Encounter (FPE): If you have already used your benefits or completed your FPE, you are generally empaneled at that facility for at least one year. You may only request a transfer during the fourth quarter of the year (October to December).
- Approved transfers from the fourth quarter will take effect on January 1 of the following year.
- Clinic Closure: You may transfer immediately if your current YAKAP clinic closes or is no longer accredited.

*We decided to provide a two (2) weeks' supply each time for medications that address hypertension, diabetes, and cholesterol to avoid depleting the supply quickly. For refills, simply come back and bring your prescription 1-2 days before the last pills run out or 1 day after, provided that the schedule is available.

*Medications not listed above will be referred to the GAMOT accredited pharmacy, if available, which will give you P20,000 yearly credit line.
